2 | WHEREAS, The members of the Illinois House of | ||||||
3 | Representatives wish to congratulate Bishop Horace E. Smith, | ||||||
4 | M.D. on the occasion of his 35th anniversary with Apostolic | ||||||
5 | Faith Church; and
| ||||||
6 | WHEREAS, Bishop Smith was born and raised on the South Side | ||||||
7 | of Chicago; he graduated from Lindblom High School and received | ||||||
8 | his Bachelor of Science from Chicago State University in 1971 | ||||||
9 | and his Doctor of Medicine from the University of Illinois | ||||||
10 | Medical Center in 1975; he completed his Pediatric Residency | ||||||
11 | with the University of Illinois Hospital in 1978 and his | ||||||
12 | Clinical Fellowship in Pediatric Hematology/Oncology with the | ||||||
13 | Children's Memorial Hospital at Northwestern University in | ||||||
14 | 1980; and
| ||||||
15 | WHEREAS, On March 28, 1980, Bishop Smith became Pastor of | ||||||
16 | the Apostolic Faith Church; under his leadership, the | ||||||
17 | congregation has grown from 200 members to 7,000; each year his | ||||||
18 | ministry provides assistance by way of food and supplies to | ||||||
19 | over 4,000 homeless families around
the Chicago area, a | ||||||
20 | scholarship program that grants more than $100,000 each year, | ||||||
21 | visitations to nursing homes,
outreach to correctional | ||||||
22 | facilities, and a JobSource program, which to date has assisted | ||||||
23 | more than 1,000
individuals in obtaining employment; and

1 | WHEREAS, In August of 2004, Bishop Smith was elevated to | ||||||
2 | the office of Presiding Bishop of the Pentecostal Assemblies of
| ||||||
3 | the World, Inc.; the P.A.W. has over 1.5 million members | ||||||
4 | worldwide, including in Africa, Asia, Europe, Australia, and
| ||||||
5 | New Zealand; his humanitarian endeavors reach beyond the shores | ||||||
6 | of the United States to support missions in all
parts of Africa | ||||||
7 | through his partnership with World Vision USA, where he serves | ||||||
8 | as a board member; under his leadership, Apostolic Faith Church | ||||||
9 | provides resources for the drilling of wells and they provide | ||||||
10 | food, clothing, education, medical assistance, and other goods | ||||||
11 | and services; over 2,000 children have been sponsored by the | ||||||
12 | church through World Vision USA; and
| ||||||
13 | WHEREAS, Bishop Smith has served as Director of the | ||||||
14 | Comprehensive Sickle Cell/Thalassemia Program at Children's | ||||||
15 | Memorial Hospital for over 20 years; he is an Assistant | ||||||
16 | Professor of Pediatrics at the Northwestern Feinberg School of | ||||||
17 | Medicine and is recognized world-wide as an outstanding | ||||||
18 | pediatrician; he has been honored by numerous organizations, | ||||||
19 | including the Children's Miracle Network, and
has been featured | ||||||
20 | in local and national newspapers, magazines, and on television; | ||||||
21 | he currently serves on the boards of
the historic Wabash YMCA, | ||||||
22 | the Family Institute at Northwestern University, and the | ||||||
23 | University of Chicago Cancer
Research Center-Community Health | ||||||
24 | Advisory Board; and

1 | WHEREAS, Bishop Smith has been happily married to Susan | ||||||
2 | (nee Davenport) since 1976; they have 3 daughters, Lauren | ||||||
3 | (Brian) Elrod,
Rachel (Courtney) Horton, and Emily (Patrick) | ||||||
4 | Green and 4
grandchildren; therefore, be it
| ||||||
5 | RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE | ||||||
6 | NINETY-NINTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we | ||||||
7 | congratulate Bishop Horace E. Smith, M.D. on his 35th | ||||||
8 | anniversary with Apostolic Faith Church and wish him many more | ||||||
9 | wonderful years; and be it further

10 | RESOLVED, That a suitable copy of this resolution be | ||||||
11 | presented to Bishop Smith as an expression of our esteem and | ||||||
12 | respect.
